Your First Visit at Generations Family Practice in Cary, NC

• Photo ID
• Insurance card
• Referral (if required by insurance)
• Medication list
• Recent medical records (if applicable) or information from last office 

Yes, after you schedule your appointment, you will need to complete your digital paperwork via the Patient Portal. After this is completed, you will review appointment confirmations and reminders.

We are located at 1021 Darrington Drive, Suite 101, Cary, NC 27513. There is free parking available on site. We request that you arrive 30 minutes early if your forms are not completed ahead of time, and 10 minutes early if they are.

You will first meet our friendly front desk staff at check-in, you will then meet with a Medical Assistant to do your initial intake and vitals. After that, you will meet with your provider for your exam, discussion, and care planning. 

Please complete your new patient forms via the Patient Portal prior to arrival.

• Self-pay fees:
• New patient and established patient self-pay fees are the same
