BRING YOUR INSURANCE CARD TO EVERY VISIT!
Beginning on July 1, 2015, all patients, new or established, coming into the office to be seen by a provider, a nurse, for labs, imaging, or allergies, will need to present their current insurance card to the front office staff at EVERY appointment. Any patient arriving for their appointment without their insurance card will be asked to reschedule their appointment. This new requirement is due to the recent shifts in all insurance policies. We apologize for any inconvenience.
While Generations Family Practice contracts with most health insurance plans, it is important to know what your health insurance and medical payment options are. Before scheduling your visit, please be sure to check with your health insurance plan directly to see if you have access to our healthcare services and learn the specifics of your coverage. The customer service/member representative with your insurance company is the best person to inform you of the level of coverage you will have as a patient. Otherwise, you may be responsible for all or a large portion of your bill. You can also find out what if any, copayments, coinsurances, and deductibles will be your responsibility.
